At enrolment, the parent/guardian of each child is required to give the immunisation status of the child to the Centre Director.


The only acceptable documentation is:

  • AIR Immunisation History Statement that is ‘up to date’,

  • AIR Immunisation History Statement indicating the child is following an approved catch-up schedule 

  • A valid immunisation certificate issued or declared by the Chief Health Officer

  • A child who is exempt because of particular family circumstances
